In 30 seconds

Phosphodiesterase-5 (PDE5) inhibitors — sildenafil, tadalafil, vardenafil, and avanafil — are first-line drugs for erectile dysfunction. They amplify a blood-flow signal the body already generates during stimulation, so stimulation is still required. Their defining danger is combination with nitrates, an absolute contraindication that can cause severe, unpredictable hypotension. Because erectile dysfunction often signals underlying vascular or metabolic disease, evaluation matters more than a reflexive prescription.

Mechanism of Action

Erection depends on nitric oxide (NO), released from parasympathetic nerve endings and vascular endothelium during stimulation. NO activates guanylate cyclase, converting GTP to cyclic GMP (cGMP), which relaxes corpus cavernosum smooth muscle and allows arterial inflow and venous trapping of blood. PDE5 normally degrades cGMP, ending the signal. PDE5 inhibitors block that enzyme, so cGMP accumulates and relaxation persists longer. This requires an intact, stimulation-triggered NO pathway; the drugs do nothing without that upstream signal, so they enhance a physiologic response rather than generate one independently.

Agents and Pharmacokinetic Differences

Sildenafil has a relatively fast onset and moderate duration; a high-fat meal delays absorption and blunts peak effect. Vardenafil is similar in onset, duration, and food sensitivity. Avanafil has the fastest onset and a relatively short duration in the class, with less pronounced food interaction. Tadalafil stands apart with a much longer duration, minimal food effect, and an approved low-dose daily regimen instead of only as-needed use, supporting spontaneity for couples who prefer not to time a pill around intercourse.

Additional Indications

PDE5 inhibitors relax pulmonary vascular smooth muscle and treat pulmonary arterial hypertension, improving exercise capacity. Tadalafil is also approved for benign prostatic hyperplasia, relaxing prostatic and bladder-neck smooth muscle to ease urinary symptoms — useful when BPH and erectile dysfunction coexist.

Contraindications and Cautions

Nitrates in any form — sublingual, oral, transdermal, or recreational nitrate compounds — are an absolute contraindication, since both pathways converge on cGMP-mediated vasodilation and combining them can cause profound, treatment-resistant hypotension. Alpha-blockers require caution, since both lower blood pressure; low starting doses and staggered timing help when both are needed. Potent CYP3A4 inhibitors slow PDE5 inhibitor metabolism, raising drug levels and adverse-effect risk.

Adverse Effects

Common effects reflect vasodilation: headache, flushing, dyspepsia, and nasal congestion. Tadalafil is particularly associated with back and muscle pain. Sildenafil can cause transient visual disturbances, including a blue-tinged color change, from PDE6 cross-reactivity in the retina. Rare but urgent events include non-arteritic anterior ischemic optic neuropathy (sudden vision loss), sudden hearing loss, and priapism (a prolonged, unwanted erection) — each demands immediate care, with untreated priapism risking permanent tissue damage.

Second-Line and Non-Pharmacologic Options

When PDE5 inhibitors are contraindicated or ineffective, options include intracavernosal alprostadil injection (a prostaglandin E1 analog acting directly on cavernosal smooth muscle), intraurethral alprostadil suppositories, and vacuum erection devices that mechanically draw blood into the penis, maintained with a constriction ring.

The Essential Clinical Point

Erectile dysfunction is frequently an early marker of systemic disease — atherosclerotic endothelial dysfunction, diabetic vascular or nerve damage, low testosterone, psychological factors, or medication effects. A thorough history, cardiovascular risk assessment, and medication review matter more than a prescription, since undiagnosed cardiovascular disease is far more dangerous than the symptom itself.
